ESY Student Registration
Gathering data and information for students who qualify for the Extended School Year (ESY) program can be a lot. Hopefully this will give you some helpful tips when completing the required ESY documentation.

Data Collection Form
Gather information throughout the school year on the data collection form- the Regression and Recoupment data of Specific IEP Goals provides a summary of skill regression and recoupment and is submitted with ESY Registration materials. You will need to take data on the students ESY goals before breaks, immediately after breaks and after twice the amount of time of the break had elapsed (for a one week break you would take gather data again 2 weeks after the break, for a 2 week break you would gather data again 4 weeks after the break.)

1066 Transportation Form
If the student requires transportation for ESY and lives within the Denver boundary, you will need to fill out this form. Select ESY Program from the type in the upper left hand corner.
